Once you’ve chosen one of our homes, there are a few steps we take to make sure the transition is as smooth and comfortable as possible for you or your loved one.
1. Referral: A referral can be made by the local authority, a family member, or directly by you.
2. Choosing Your Home: After visiting our homes and deciding which one feels right, we’ll move on to the next step.
3. Pre-Admission Assessment: Our experienced senior care team will conduct a detailed assessment to ensure we can meet all your loved one’s needs.
4. Confirmation: We’ll confirm that we’re able to provide the right care and support.
5. Funding Agreement: We’ll discuss and confirm the financial arrangements.
6. Admission: Once everything is in place, your loved one can move in.
Before your loved one arrives, a member of our senior care team will carry out a thorough pre-admission assessment. This is an important step to make sure we understand your loved one’s needs, likes, and lifestyle. Whether the assessment takes place at home, in a hospital, or another care facility, we want to know as much as possible to ensure the transition is seamless and stress-free.
All the information shared during this process is kept confidential but will be shared with our care team to help create a personalized care plan tailored to your loved one’s needs. This ensures they’re supported in the best way possible from day one.
Once we’ve confirmed funding, we’ll finalize everything, and your loved one can move into their new home. We understand that timing can be important, so we aim to complete this process quickly, often within just a few days.
